(To be filled by only those desiring grant for construction or extension/addition of library building.)

1. Name and address of the library.          

2. Description of the proposed construction.

3. Total estimated expenditure on the proposed construction. (Total estimated expenditure should not exceed Rs. 4.00 lakhs)

4. How long it will take to complete the construction? (Organisation must complete the construction within two years from the date of first instalment)

5. How much assistance is required by the organization to complete the construction? The amount asked for should not exceed 75% of the total estimated cost? (Shown against Sl. No.-2 or Rs. 3.00 lakh whichever is less)

6.Whether the plan for the proposed construction is approved by the local body viz. Corporation, Municipality, Panchayat and Notified Area Authority? (If the proposed construction is a part of higher plan the portion proposed to be covered under the proposed scheme should be demarcated with Red Ink on the body of the plan, approved to be attached)

1. Please indicate whether the total estimated cost of the construction is based on state PWD/ CPWD prevailing rates and duly authenticated by a registered firm of Architect/ Building Engineer/ PWD/ CPWD. (Original estimates to be attached)

2. In case of proposed construction of the ground floor –

1. Total cost of construction up to plinth level.
2. Total cost of construction up to roof level.
3. under the proposed scheme should be demarcated with Red Ink on the body of the plan, approved to be attached)

      iv.In case of construction for works of          other than ground floor –

1. The cost of construction up to lintel level.
2. The cost construction when the roof has  been laid.
3. The cost of construction of remaining work.

7. Whether the library is at present housed? Whether it is in a rented accommodation or in its own building and whether it is a temporary construction or a pucca construction?

8. Whether the proposed construction will be adjacent to or on the existing building.

9. Does the plot of the land on which the building is proposed to be constructed belongs to the library? Mention the total area of the land with cost there of.

  (English version of the deed of land along with a certificate from Government pleader be attached)

10.Whether the organization is in a position to meet the balance of the expenditure i.e. 25% of the total estimated expenditure or the balance thereafter for the construction of the library building? If so the source of the receipt be mentioned.

11. Declaration
